Please take the time to read these terms of service. They are important for you to fully understand and voluntarily agree to, before commencing consultations.

The Lifestyle Medicine for Women commitment to you

  • Lifestyle Medicine for Women strives to provide excellent care and service in your journey towards a healthier lifestyle.
  • Where you do not feel this has been achieved, you are invited and welcome to discuss this with me directly and we shall communicate amicably and openly to seek a resolution together.

Your commitment to Lifestyle Medicine for Women

  • Please fully engage with the healthy lifestyle consulting process to the best of your ability. This will give you the greatest opportunity for strong results.

The Lifestyle Medicine for Women Scope of Practice

  • Provision of ‘healthy lifestyle consulting.’
  • This includes the assessment of current healthy lifestyle behaviours, followed by education, advice, coaching and support to assist in the achievement of improved healthy lifestyle behaviours that can help to prevent and/or manage lifestyle related health issues.
  • Healthy lifestyle behaviours include food, movement, sleep, stress, emotional and social wellbeing, alcohol and tobacco.
  • NB. Alcohol and tobacco use will be assessed and discussed as an essential part of the healthy lifestyle consulting process. However, I do not treat addictions specifically and no medications will be prescribed. If focused help for alcohol and tobacco use seems required I will advise you to address this with your Dr or other relevant organisation. It will be your responsibility to then follow this advice.
  • NOT provision of ‘medical care.’
  • I am not working as your Dr or GP. Though an established medical history will be obtained as relevant to the healthy lifestyle consulting process, there will be no seeking symptoms for undiagnosed illnesses, no physical examination, no physiological measurements (such as blood pressure), no requesting investigations (such as blood tests or imaging), no making new medical diagnoses, no changing medical diagnoses, no prescribing or changing of medications.
  • The medical care of all acute and chronic medical conditions will remain under the care of your Dr or other healthcare professional(s) as usual.
  • If, during consultation, I note anything of concern that I feel you need to address with your Dr or other healthcare professional(s), I will inform and advise you of this. It will be your responsibility to then follow this advice.
  • I am happy to share correspondence of our work with your Dr or other healthcare professional(s) if you wish.

Outcomes are not guaranteed

  • While people generally experience greater health and wellness as a result of a healthier lifestyle, I cannot promise or guarantee specific results or protection from future illness, due to the large number of variables that can impact on your health.

Payment

  • It is requested that each consultation fee is paid within 48 hours of the consultation.
  • You will be sent an email invoice with a link to payment.

Cancellations

  • Lifestyle Medicine for Women requests a minimum of 24 hours notice for appointment cancellations if at all possible.
  • Payment will not be requested for a first cancelled appointment with under 24 hours notice, as I realise that things sometimes happen.
  • Payment WILL be requested for a second cancelled appointment with under 24 hours notice and prepayment will be requested for further appointments.

Late and missed appointments

  • If you arrive late for an appointment, it may not be possible to extend the length of your appointment but we will do what we can within the time remaining. This is in order to be fair to other clients.
  • Payment will not be requested for a first missed appointment, as I realise that things sometimes happen.
  • Payment WILL be requested for a second missed appointment and prepayment will be requested for further appointments.

Data protection policy essentials

  • Lifestyle Medicine for Women has an ethical and legal obligation to protect your personal data.
  • I work to the best of my ability and knowledge to ensure full compliance with the UK GDPR (general data protection regulations).
  • I will ask for consent before collecting your personal data.
  • I will collect only the data which is relevant to, and for the purposes of, consultations and/or to update you regarding Lifestyle Medicine for Women services and information.
  • Your personal data will be in fully secure, UK GDPR compliant platforms at all times:
    o Consultation data will be kept, and online telehealth sessions will be conducted (not recorded), on the Practice Better platform (www.practicebetter.io)
    o Website hosting and management is by Healthhosts (www.healthhosts.com)
    o Email newsletters will be delivered via Mailpoet (www.mailpoet.com)
    o If you email me directly at geraldine@lifestylemedicineforwomen.co.uk I will assume your consent in sharing your email address to contact me but will not add you to any email list or further use your personal data without your consent.
  • Your personal data will not be shared with any other third parties.
  • Your data will only be kept for as long as is necessary.
  • You can unsubscribe from Lifestyle Medicine for Women services at any time.
  • A copy of the full Lifestyle Medicine for Women data protection policy is available on request. I am also happy to answer any queries.

Confidentiality

  • Your confidentiality is paramount. All personal information will remain confidential unless in the following circumstances:
    • You request or give consent for your personal information to be shared
    • The sharing of your personal information is requested by law or in response to a court order
    • The sharing of your personal information is justified in the public interest
    • Even in these circumstances, Lifestyle Medicine for Women will always attempt to seek your consent and/or advise you before sharing your personal information wherever possible and practicable.

Exclusion of liability

  • You understand that any lifestyle changes undertaken at Lifestyle Medicine for Women are undertaken of your own free will. You accept that the ultimate responsibility for your health care is your own and that Lifestyle Medicine for Women is here to support this. You understand that your practitioner reserves the right to determine which cases fall outside of their scope of practice, in which event you will be redirected if possible.
  • Lifestyle Medicine for Women shall not be liable for, or accept any responsibility for, any injury, loss or damage howsoever sustained by any person or persons arising out of any consultations delivered by Lifestyle Medicine for Women, or in any way whatsoever that does not arise from any negligent act or omission of Lifestyle Medicine for Women.
